Appointment Cancellation Policy

We strive to render excellent care to you and the rest of our patients. In an attempt to be consistent with this, we have an Appointment Cancellation Policy that allows us to schedule appointments for all patients. When an appointment is scheduled, that time has been set aside for you and when it is missed, that time cannot be used to treat another patient.

Our policy is as follows:

We require that you give our office at least 24 hours’ notice in the event that you need to reschedule your appointment. This allows for other patients to be scheduled for that appointment. If you miss an appointment without contacting our office within the required time, this is considered a missed appointment. The following are missed appointment fees for each type of service:
– Medical visit: $50
– Physical Therapy/Massage: Varies
o If you purchased through Groupon, you will forfeit full value of your massage
certificate
o If you purchased through our office directly, you will forfeit full value of the
massage that you have purchased
– Auxiliary medical services(these services include all other appointment types such as venipuncture, glucose testing, PT/INR and other Medical procedures offered in our office: $30

Please note, all massage appointments must provide credit card and prepay for their massage at the time of the booking. All Groupon customers, please have your voucher number ready at the time of the booking.

Additionally, if a patient is more than 5 minutes late without prior notice for a scheduled appointment, we will consider this a missed appointment and cancellation fee will be charged appropriately to the service scheduled.
